Household Income and Coverage Levels
Coverage under the CDCP is based on your adjusted family net income, determined from your most recent tax return.

How to Apply for CDCP
Applying for the Canadian Dental Care Plan begins with confirming your eligibility through the Government of Canada. During the application process, you may be asked to provide information such as your Social Insurance Number, income details, and recent tax return information.
Once approved, you will receive confirmation from Sun Life that includes your member card, coverage level, and benefit start date. After your coverage becomes active, simply
contact Altus Dental to schedule your appointment and discuss your treatment needs.
