Get Started with CDCP Coverage

Household Income and Coverage Levels

Coverage under the CDCP is based on your adjusted family net income, determined from your most recent tax return.

Adjusted Family Net Income CDCP Covers You Cover (Co-payment)
Under $70,000 100% of eligible services at CDCP rates 0% (you may still face additional fees)
$70,000 – $79,999 60% of eligible services at CDCP rates 40% of CDCP rates
$80,000 – $89,999 40% of eligible services at CDCP rates 60% of CDCP rates

Note: Altus Dental may charge above CDCP fee guidelines. Any additional costs not covered by the plan are the patient’s responsibility. We recommend confirming coverage and fees with our clinic before beginning treatment.

How to Apply for CDCP

Applying for the Canadian Dental Care Plan begins with confirming your eligibility through the Government of Canada. During the application process, you may be asked to provide information such as your Social Insurance Number, income details, and recent tax return information.


Once approved, you will receive confirmation from Sun Life that includes your member card, coverage level, and benefit start date. After your coverage becomes active, simply contact Altus Dental to schedule your appointment and discuss your treatment needs.
